Background:
The Charter medical Group, Ireland’s foremost centre of clinical excellence in geriatric medicine in the community is expanding its scope of operations and opening a medical homecare’ division. This service will be unique in Ireland as it will for the first time provide fully integrated medical care to compliment homecare packages. The clinic’s team of care assistants will be supported by a fully integrated nursing team who will provide 24/7 triage care and a team of geriatric consultants and registrars based at Charter’s clinic at Smithfield, Dublin.
The Person
We are looking for a suitable candidate to head up the ‘start up’ phase of the new division. The person needs to have a clinical/nursing background and demonstrate exceptional leadership and patient care skills.
The Role
The successful candidate will be responsible for:
- developing strong sustainable relationships with healthcare professionals (especially PHNs, social workers, hospital staff, etc) and other referral sources
- enhancing existing relationships with our existing referral providers
This could include: information calls, one to one meetings, group presentations to provide information regarding available services to Hospitals, Nursing Homes, Doctors, Health Centres and the general public.
- conducting face to face service calls to clients and decision-makers as required
- identifying needs for other services and assist with their development
- The provision of a high standard of care by our care assistant team to our patients and families
Responsibilities
- Encourage recognition of the patient as an individual; ensuring their healthcare needs and comfort are given priority
- Oversee the delivery of care together with all members of the multidisciplinary teams.
- Communicate with relevant teams regarding on-going operations.
- Communicate with families on an on-going basis.
- Attend / chair meetings as required and keep staff briefed on developments in the division.
- Oversee the organization and management of the Care assistant team
- Oversee and co-ordinate the quality and safety systems including all aspects of risk management
- Oversee financial expenditure and value for money.
- Be responsible for the recruitment, development and supervision of staff
- Facilitate the arrangements necessary and participate where appropriate, in education and training of other staff as the need arises
- Be responsible for the development and implementation of policies and procedures
- Liaise with external organisations in order to promote and maintain good working relationships and high standards of care and service
- Motivate team members, by agreeing goals and objectives, through performance review
The successful candidate will ideally be a RGN, with evidence of continuous professional development. You will have minimum three years post graduate experience in gerontology with a minimum of two years experience in Care of the Older Person at CNM level and you will have an in-depth knowledge of the ageing process. You will be able to demonstrate sound knowledge and clinical nursing practice together with future trends in care of the elderly.
